On-site SEO and Off-site SEO. On-site SEO means page design, layout, keywords, titles, etc. Off-site SEO is links to your website from the right places. I have one keyword that made it to the second 5, and it has been fluctuating between 5 and 10 for the past 6 months without any intervention from me.
